You don't put the fertilizer on the same square as the crop, you have to put them next to the crop or in the same furrow. Like that:

I have not tried that spiral pattern yet, no...man, that's 24 fertilizers, so 24,000g... heh, maybe in Year 2.
But I have dug some long squiggling trenches that I put like 6 fertilizers in, and it definitely raises the crops more than just a single fertilizer would. (I use that for my crop festival crop, and sometimes to get the 1-star items that you need for some B-level requests).
Fertilizer lasts a ridiculously long time if it's only got one crop sharing the trench with it. I mean - those 6 I was talking about fertilizing my crop festival crop? They lasted a whole year.
The only way fertilizer really gets wasted is if you pick the crop, then leave the crop spot blank long enough that it reverts to untilled soil, breaking the trench and leaving you with a solid trench full of fertilizer that doesn't do anything. Then you have to destroy one of the fertilizers with your hammer in order to open up a spot to plant a crop in that trench again... (urg, had to do that once...so annoying...)

I believe fertilizer provides one quality point per day, with each star being 60 points. I have grown crops in a trench with 20 fertilizer, and things like Carrots or Onions end up about one star better. Since fertilizer only works when they're growing, not fully grown, it makes sense to only water things once per day, if you're not in any rush to get them.

Did you put the fertilizer in when the crop was seeds or growing? I think it only affects the crop when you just plantit and its still in seed form.
