Is it possible to get a 5-star version of every item, even things like weeds? If it's possible, how do you get a 5-star weed, or a 5-star insect?

Yes. Star ratings increase every year. I'm in year 3 and I can pick up 3 star weeds, sticks, bugs, etc.
